Donations

Sumner Hospice is a 501(c)(3) not-for-profit, community-based organization. Donations allow us to provide end-of-life care to qualified patients and their families regardless of their insurance coverage or their ability to pay. In addition, ongoing donations allow Sumner Hospice to utilize a broad range of state-of-the-art clinical, psycho/social, spiritual and volunteer services and patient programs

Contributions take many forms. You may elect to make your donation in the form of cash, a check, or a credit card; stocks, bonds, mutual funds, or life insurance proceeds; personal property or real estate holdings; and annuities, wills and trusts. In addition, we accept in-kind contributions for a variety of goods and services.

Each donation is formally acknowledged in writing with the letter serving as the receipt for tax purposes. In addition, all memorials and tributes are acknowledged to the honoree or their family, though the amount is not disclosed in the letter. When an honoree’s memorials and tributes meet $1,000.00, the honoree’s name is added to our Memorial Wall.

Donations made be made:

  • By Phone: (615) 452-4009, Sumner Foundation
  • By Mail to: Sumner Foundation
    Post Office Box 1558
    555 Hartsville Pike
    Gallatin, TN 37066

Please include the donor’s name and address, whom the gift is in honor/memory of, and the honoree’s family’s name and address. This will ensure proper acknowledgement. For more information, visit the Sumner Foundation website.

Planned Giving

Contributing through planned giving, estate planning or bequests creates a legacy of community service for the donor. We suggest you consult an estate-planning attorney to thoroughly understand your various options in order to make informed choices and decisions. An estate-attorney will also be able to advise you on the preparation necessary (e.g., your assets and liabilities, selection of a trustee(s), and destination of your gift) to ensure your wishes are properly carried forth and conform to state law.

If you wish to make a contribution through a planned giving instrument, please contact Sumner Foundation at (615) 452-4009 or karen.rudzinski@sumner.org.

Every gift is significant. Whatever method you choose to express your support, you are assured your donation will positively impact the quality of life of our patients and their loved ones, your friends and neighbors, in the community.

Please note the IRS determines an organization’s 501(c)(3) status and all contributions that are not made in exchange for goods or services are tax deductible. Please consult your financial advisor for details relevant to your personal situation.
