Sumner Homecare Admission Criteria

To qualify for home health several criteria must be met:

• patient must be homebound
• patient must be under the care of a physician
• the care must be provided in the patient’s place of residence (home, assisted living facility, group home or a family care home)
• the services must be skilled and needed on an intermittent basis

Referrals for Homecare

Most referrals come from a hospital, physician, rehabilitation or skilled nursing facility.
