Sumner Hospice Admission Criteria
Admission to the Sumner Hospice program is made upon recommendation of your physician, based upon your needs. Normally, appropriate candidates for hospice are patients:
- With a life expectancy of 6 months or less if the illness runs its normal course;
- Who desire palliative treatment;
- Who want to stay at home as long as possible; and
- Who have a primary care person
On admission, our nurse will visit you or your family to discuss Hospice services, assess your immediate needs and recommend a plan of care.
Referrals for Hospice
Most referrals are made by a physician or a hospital discharge planner. Family members and patients can also make referrals.

Sumner Hospice services include:
- Nursing
- Medical Social Services
- Therapy Services
- Chaplain
- Home Health Aide
- Nutrition Support
- Volunteer Services
- Bereavement
All services are provided under the direction of a physician and/or the Hospice Medical Director. Arrangements will be made for hospice approved medications, medical supplies and equipment, as appropriate.
